I was having a similar issue with my 2GB card using Vista Business 32bit on an HP NC6400 laptop w/built-in card reader. I would get the "You Must Format This Volume" message, & when I tried, it would say that "Windows was unable to format this volume"
I could boot into linux & use the card no problem! It also worked in XP on other PCs.

After much hunting, I found this thread regarding a 4GB card:

None of the new drivers made a difference, but making the following change to the registry key worked (as in above post)!
Under HKEY_LOCAL_MACHINE\SYSTEM\CurrentControlSet\Services\tifm21\Parameters
change the "SDParam" dword value from decimal "16" to decimal "1" & reboot.
