Q1.....Do you have automatic MS Windows Updates, and do you install them....the critical and important?

Q3.....I am aware that a Java script nasty can embed itself right in the Drive and location that you mention..... D:\Recycle Bin, and that the Windows Live OneCare Safety Scanner has successfully located it and given details at the scan finish on how to quarantine and remove it. Both are free to use, worth a try, and often overlooked as useful tools.
By the way, the latest Version of the Malicious Software Removal Tool, is due for release with other MS Windows Updates, on Tuesday this week.
