BSOD NVIDIA Graphics Driver

I'm running vista home premium 32 bit.
Recently I suddenly get yellow lines across the screen whenever I try to watch a video or play a game and the computer crashes, and when i force a restart i get the BSOD. Sometimes the it goes straight to blue screen whilst rebooting.
Windows says that 'NVIDIA Graphics Driver has stopped working properly', and I've tried all the microsoft solutions such as reinstalling graphics driver, uninstalling the video drvier and then rebooting, and checking for new updates but nothing has happened. My display adapter is NVIDIA GeForce 8400 GS.
I've also run a sfc/scannow scan to no avail.
I've attached the minidump folder and would greatly appreciate any help, thanks

In short, try everything there - if it still crashes then try a new video card.
