The junk filter does work, but obviously not on the precise message
you're getting. It doesn't learn like some do. We're told that
monthly updates will be forthcoming via Windows Update in the
future.
When you say 'same junk mail over and over', how is it 'same'? Same
sender, same subject, same content? What kind of account - POP3,
IMAP, HTTP? If it's POP3, you can set up a rule to filter it out. If
it's an HTTP account, you can use the filters at Hotmail (on sender
and subject) to deal with it.

As noted, there can be many reasons. Often the SPAMers like to use the
same message, but send it from a slightly different address or make some
other small, easily overlooked changes to beat out filters.
