Context Menu Item

I have created a context menu entry in the registry to add a functionality
that I want to show up when I right-click all items including Control Panel
icons, works for the most part. The issue I have is that it does NOT display
for Control Panel icons nor does it work for Windows ADUC menu items. Below
is the entry I have added:

hkey_classes_root\*\shell\Vista RunAs\command

The entry just simply isn't available for the system type of shortcuts. Is
there a place that I can put a context menu registry entry that will display
over ANY icon I click...all file objects? Thanks in advance for your
assistance.
