Well according to Google the Fault Module , OpHookSE4.dll appears to be part of ScanSoft OmniPageSE4.
So maybe you have that installed and giving you problems.

Thank you. I uninstalled OmniPageSE4, and my old scanner software and my
errors disappeared. Thanks very much.
