Vista - Hibernation only works once per restart cycle

Hibernation only works once per restart cycle

I hibernate my PC and it wakes up with no issues, yet if I hybernate again it
won't even go to sleep (it freezes up and I must manually turn it off and
then on again).

This cycle repeats forever, one hibernate and then a restart is required.
Any ideas on what this could be?
