How to reduce the number of items in my context menu

I'm looking for a way to reduce the number of items in my contextual menu.

I have a utility that lets you turn them off, which is great, except that I do use some of them from time to time. What I would like is something that:

1) Could group them in submenus (which should activate only by clicking on the group item)
2) Would let me hide a good part of them unless I press either Ctrl or Alt.

Is there such a beast?

I sure hope so, I am at a point where it takes 5 seconds for the menu to appear and often it freezes the program I am working on for longer than that.
