screen split into 6 tile size screens

Hi guys, two months ago i was watching a video on youtube and 5minutes later the laptop freezes. When i restarted the laptop, the screen split into 6 smaller screens with each screen being a carbon copy of the other. They all responded simultaneously for key I hit on the keyboard. I restarted the computer in Safe mode, scanned and Restored the laptop at a previous date but none of these helped. Today, which is 2months after i experienced this issue, i turned on the laptop and bam!! the problem was fixed. After 10minutes, the laptop freezes and the blue screen appeared with this technical information: "***Stop:0x00000124". So i shut down the laptop using the radio button. When i turned it on again the 6 tile screens reappeared again but this time the computer stayed on a gray screen forever. What should i do to fix this?
