Problem: Dual monitor setup does not wake up from Sleep.

When waking up from sleep mode with a dual monitor setup, the computer does not display anything either monitor and has to be manually shut down. The problem does not seem related to motherboard settings such as Wake-On PCI-E, Spread Spectrum, or changing the suspend mode between S1/S3/S1 & S3. The cause of this problem began from a video card upgrade from a BFG 7600GT to an Asus HD4850.

The problem can be repeated with the following steps:
1.) Load Windows Vista after shutdown and don't log into a user account
2.) Wait at the login screen and then time 15-30 seconds before pressing the power button (assigned to sleep on press)
3.) Wait 15-30 seconds before pressing the power button again
4.) If computer woke-up, check off success on paper and repeat steps 2-3. If not, follow to step 5.
5.) Hold the power button and shut down the computer manually. Logged how many tries before it "craps out" from wake-up.

System info:
Windows Vista Ultimate 32-bit SP1
3.1Ghz AMD X2 5000+
Asus M2A-VM Motherboard
4GB DDR2 800Mhz RAM (OCZ + Patriot)
Asus EAH4850 512MB (with two DVI to VGA converters) Catalyst 8.8
Samsung SyncMaster 710M LCD
Samsung SyncMaster 765MB CRT

320GB Seagate 7200.10 SATA HDD
OCZ StealthXStream 600W PSU

Results:
The expected result would be to have the computer wake-up no matter how many times the user put it to sleep mode from the login screen. The actual results are that it varies on how many "Sleep sessions" occur before the system becomes completely unresponsive and has to be shut down. My results showed that performing the previous steps @ 15 second intervals it would crap out between the 1st and 3rd wake-up attempt (tested 5 times). Performing them @ 30 second intervals would also crap out between the 1st and 3rd wake-up.

So far there's absolutely no link between how long I wait before waking-up and the number of tries it takes to crap out the Sleep system. Its still totally random. Any help?

I've honestly tried all workarounds and fixes and have had zero luck with the problem. I also followed a tutorial by Shawn and had no luck again. I noticed in another thread the user had the same problem as me and coolchris mentioned there was a fix for this ages ago, but never got back to the thread. And yes its safe to assume all my drivers are latest from the manufacturers of my parts. Here's a little more detail from the notes I've been keeping on my issue:


1.) Notes related to my issue:
  • BIOS: Wake-on PCI-E - Disabled, result: fail
  • BIOS: Disabled spreadspectrum, result: fail
  • HARDWARE: Removed 2nd monitor from the setup, result: fail
  • BIOS: Changed ACPI setting between S1 & S3, to just S3, result: fail
  • OS: Restored Hibernation Files through command prompt, result: fail
  • OS: Removed all startup programs through msconfig, result: fail
  • OS: Uninstalled Asus Smart Doctor monitoring software, result: fail
  • OS: Disabled wake-up through mouse in Device Manager, result: fail
  • OS: Enabled Hibernation, disabled Sleep through power management, result: pass
