Help! Reformatting Incomplete with Errors

Hi,

I need help, it's my first time reformatting my laptop and I'm completely stuck. I put the reinstallation Vista DVD and it went through the steps until it reached the screenshot below. At the last step "completing installation" there was an error ( I can't remember the error) and it began to restart. I'm now stuck on the window's log in screen, where it's asking for username and password. This was never set up in the past, and it didn't prompt me to set it up before reaching this log-in screen. Does any one know what I need to do?
