To add programs, click on Browse and browse to the .exe for the program you want to use.
I don't know how to delete programs. Most likely they're listed in a html or other file somewhere for default programs. Maybe in the registry but didn't look.

If you follow this tutorial: Add "Open with Notepad" to the Context Menu for All Files - How-To Geek
You will add an option, "Open with Notepad", to the context menu when you right click on any file. Once you've got it set up, it's nice and simple to use.
