Based on information given, it may be your anti virus. You can make a system restore point to go back, just in case. Then uninstall and replace with Microsoft Security Essentials and a once a week scan with the free Malwarebytes.

I cant answer that. If its software, and you post the information that I requested, we can fix it very easily, but yes a format and reinstall would solve it.
If its hardware, maybe memory, you must get the computer fixed.

The most imporant file is the mini dump. First just get it for yourself, then we will try to send it here.

Sorry my friend the mini dump is not included.
Lets try this uninstall your anti virus and relplace with Microsoft Security Essentials. That is a possible cause. You can make a system restore first, in case you want to go back.
